Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

Position Summary:
The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services. As an independent professional, the Nurse Practitioner will work to full scope of practice to contribute to clients’ health providing assessment, prevention, education, rehabilitation, and therapeutic support services consistent with the standards of care outlined by the College of Nurses. We are seeking a Nurse Practitioner who is committed to community-based, client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.

Salary: 59.26-71.24
