We are thrilled to invite you to join our NEW Interprofessional Primary Care Team, aimed at improving healthcare access and attachment for residents of Thorncliffe and Flemingdon Park neighbourhoods. Our services will be extended to various locations offering access clinics with expanded hours to better serve our community.
Our enhanced care team will offer a range of services focused on illness prevention, health promotion, health education, comprehensive primary care, social prescribing, and population health. initiatives. This will be done through a team comprised of MDs, Holistic Intake and Navigation Counsellors, Peer Support Workers, and RNs. These services will be accessible through various points in the community, making it convenient for residents to receive care. Background:
Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.
Heath Access Thorncliffe Park (HATP) is committed to providing coordinated Health & Wellness services to the Thorncliffe Park community. It is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. HATP is an innovative partnership between Flemingdon Health Centre (FHC) and TNO The Neighbourhood Organization (TNO) funded by the Ontario Health- Toronto region.
Position Summary:
The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services. As an independent professional the Nurse Practitioner will work to full scope of practice to contribute to clients’ health providing assessment, prevention, education, rehabilitation and therapeutic support services consistent with the standards of care outlined by the College of Nurses. We are seeking a Nurse Practitioner who is committed to community based, client-centred care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.
